Request for home extraction analysis

If you wish to request an extraction at home, please fill out the following form and we will contact you as soon as possible.

We remind you that you must have all the administrative documentation for the home analysis extraction:

• The flyer or request prescribed by the doctor from the medical chart of your insurance company must request the Home Analysis service.
• You must have a specific authorization from your health insurance company in order to perform the service.

In the event that you do not have the documentation in order, you must pay us the amount of the service at your home.
